    Visitor Guidance for Lakeland Community Hospital Inpatient and Clinics
    Effective October 2, 2020

    COVID-19 and Persons Under Investigation (PUI) for COVID-19

    • No visitors allowed except in end-of-life situations Non-COVID 19 Inpatients

    • One caregiver will be allowed in a 24-hour period. The Caregiver will not be allowed to rotate with any other Caregiver during this 24-hour period in consideration of the need to limit exposure.
    • Caregiver must follow the following Guidelines.

    o Wear your own face covering upon arrival to the facility and must wear it at all times.

    The face covering must cover your mouth and nose at all times.
    o There are no exceptions to wearing face coverings for the health and safety of our

    patients, caregivers, and staff.
    o Agree to practice appropriate hand hygiene and limit the touching of surfaces in patient

    care areas.
    o Stay in room: Caregivers are asked to stay in the patient’s room and not leave the unit

    or roam the hospital except for basic personal needs (e.g. food, restroom)
    o Comply with Entrance screening: LCH will continue limited and monitored points of

    entry, with active Caregiver screening upon entry.
    o Entrance will be overseen by our Admitting department with Nursing and Security

    personnel.
    o All Caregivers must sign in at the entrance to help us track who is in our facility.
    o Entry will be denied to anyone who screens positive for COVID-19 such as temperature

    above 99 degrees, cough, or shortness of breath.
    o Caregiver must wear visitor badge at all times. These will be provided at both entrance

    locations. Caregiver must return badge when exiting facility.
    o Designated entrances will only be open to Caregivers between 8 am and 7 pm.

    Caregivers as defined above will not be allowed to enter after 7pm. Any emergency need should be escalated to the Admitting department and an administrator will be contacted on your behalf.

    End-of Life Guidance

    • For non-COVID-19 patients in which death is reasonably anticipated within 24-48 hours, two Caregivers (meeting the above requirements) will be allowed at the bedside.

      o CaregiversmayrotatewithotherCaregivers

    • For COVID-19 and Persons Under Investigation (PUI) for COVID-19- only one Caregiver will be

      allowed at the bedside pursuant to Physician approval

    o Caregivers will be required to follow all instructions from clinical staff related to hand

    hygiene, PPE, entry/exit, length of time with patient, etc.

    Emergency Department

    • One Caregiver may accompany a patient and remain in the patient’s room at all times with a face covering over the mouth and nose.
    • If the patient is under the age of 14, two Caregivers may accompany the patient but must remain in the patient’s room at all times with a face covering the mouth and nose.
    • Failure to follow these guidelines will result in immediate removal from the facility in order to protect the health and safety of patients, Caregivers, and staff.

      Inpatient Psychiatry Unit

    • Under the recommendation of the treatment team, one visitor may be allowed by appointment only during established visiting hours. Telephone and video conferencing is available and encouraged.

    Rural Health Clinics (Boyde J. Harrison, MD, Lakeland Family Practice, and Haleyville Pediatrics)

    • Visitors will not be allowed except for those meeting a compassionate care qualification. One Caregiver/Support person will be allowed to accompany the patient only when one of the following circumstances is met:

    o Patient has cognitive/memory disorder
    o All Caregivers must wear a face covering. The face covering must cover your mouth and

    nose at all times.
    o Symptomaticscreeningwillbeconducted
    o Proper social distancing at 6 feet is maintained at all times within the facility.

  • Haleyville City. Council

    Monday, September 21 – The Haleyville city Council met with all members present and approved the general operating bills for August in the amount of $465,114.06. The Council also approved a resolution to apply for Alabama Department of Transportation (ALDOT), Alabama Transportation Rehabilitation and Improvement Program II (ATRIP II) funds for the addition of a left turn lane at the intersection of SR-195 and 20th Avenue during fiscal year 2021. The City of Haleyville will request ALDOT waive any requirements to relocate utilities or that ALDT pay for any utility relocations.

    A Resolution was approved for the following streets considered for the 2020 Fall Paving Project:

    • 12th Avenue, between 29th Place and 11th Avenue
    • Briarwood Drive
    • Patch at Culvert on North Hills Drive
    • Patch at 34th Street
    • Hill Top Subdivision (partial)
    • Acting City Engineer, Calvin Cassady, P.E., has reviewed these streets and provided cost estimates totaling $205,238.00 to complete this work. These projects will be bid out.

    The Council approved a pay scale for custodian pay, starting at $11.00/hour.

    The Government Health Insurance will increase 5.5% for 2021. The Council approved a motion for the City to absorb the Health Insurance increase for City employees.

    The Council approved an ordinance to ratify an existing privilege license tax and to authorize the pledge of a specified portion of the proceeds thereof for the Healthcare Authority of Haleyville and Winston County. The Trust Indenture by the Authority.

    The Authority has advised the City that the Authority needs to issue revenue bonds (the “Public Health Care Bonds”) pursuant to the Authority Law to provide for capital improvements to, and the operation and maintenance of, the public health care facilities of the Authority. Regions Bank will be the trustee.

    The Council approved $1495 for a new web site constructed by Traci Snoddy of Double Springs.

    The next scheduled Haleyville City Council meeting will be Monday, October 5, 5:30 p.m. at the Haleyville City Hall.

  • Haleyville Business Licenses

    The City of Haleyville has adopted the Business License Code which is in effect within the City limits and the Police Jurisdiction.  We have received some questions about who is required to purchase a license and to collect sales tax on sales.  To help explain the obligations of people selling merchandise or services within the City and Police Jurisdiction, please observes these requirements and answers to frequently asked question.

    1.  The Levy of the Business License reads as follows:

    Ordinance #2007-11, Section 1.

    Pursuant to the Code of Alabama, the following is hereby declared to be and is adopted as the business license code and schedule of licenses for the municipality for the year beginning January 1, 2008, and for each subsequent year thereafter. There is hereby levied and assessed a business license fee for the privilege of doing any kind of business, trade, profession or other activity in the municipality, or the police jurisdiction, by whatever name called. 

    The Business license is required for anyone engaging in doing business in Haleyville regardless of what I call the activity.  For example, if I am selling merchandise through Facebook or other social media in Haleyville or the Police Jurisdiction, I must have a business license and comply with the reporting, assessment and collection of sales tax or other fees required by law.

    Questions have also been raised as to whether this is a requirement for “Yard Sales.”

    The answer is that, if I am engaging in the selling of new or used items that are purchased or collected for resale, I must have a Business License and I have to collect and submit sales tax to the City.  The name I call the activity does not override the nature of what I am doing.

    This type of activity is covered under the definition of a “Peddler” and would purchase a license based on the amount of gross sales under “Retail.” Continue reading

  • Absentee Ballot for General Election

    Tuesday, August 18, 2020 – MONTGOMERY – Ahead of the November 3 General Election, Secretary of State John H. Merrill reminds Alabama voters that there are 72 days left to apply for an absentee ballot.

    In order to protect the safety and well-being of voters, Secretary Merrill encourages those who are concerned about contracting or spreading the coronavirus to apply for and cast an absentee ballot.

    Absentee ballot applications can be downloaded online or requested by visiting or calling the local Absentee Election Manager’s office.

    Voters may also contact the Secretary of State’s Office at (334) 242-7210 to request an absentee ballot application.

    Due to the declared states of emergency, any qualified voter who determines it is impossible or unreasonable to vote at their polling place shall be eligible to check the box on the absentee ballot application that is most applicable to that individual. In the case none of the boxes are appropriate, voters can check the box which reads as follows:

    “I have a physical illness or infirmity which prevents my attendance at the polls. [ID REQUIRED]”

    For the November 3 General Election, the deadline to register to vote is Monday, October 19, the deadline to submit an absentee ballot application is Thursday, October 29 the deadline to return an absentee ballot to the Absentee Election Manager is the close of business Monday, November 2, and the last day to postmark an absentee ballot is Monday, November 2.

    Voters who are eligible to vote pursuant to the Uniformed and Overseas Absentee Voting Act will have until Tuesday, November 3 to postmark an absentee ballot.

    Due to recently witnessed delays with the United States Postal Services, Secretary Merrill encourages voters interested in returning their ballot by mail to go ahead and make application for their absentee ballot. As a reminder, Merrill worked with the Legislature last year to pass Act 2019-507, allowing voters the opportunity to return their absentee ballot by commercial carrier in addition to U.S. mail.

     

  • Two Injured in Crash

    A two vehicle collision just after 9 a.m. Tuesday morning injured both drivers. The crash was between a Hyundai driven by Sybil Hill, 73, of Haleyville and a pickup driven by Bryan Crumpton. According to Crumpton, he was driving north on 12th Avenue thru the Highway 195 intersection when he was struck by the Hyundai which was headed south on 195. The impact on the drivers side of Crumpton’s pickup knocked it into a power pole, bringing down two traffic lights. Crumpton, who was covered with blood, left the scene in another pickup. Hill was transported by ambulance to a Shoals hospital. Neither driver was critically injured. There were no passengers in either vehicle.

    The crash is being investigated by the Haleyville Police Department.
